Lord vs Darth?

I'd agree - just as the Jedi use 'Master' to refer to any higher-ranking/higher-placed Jedi, so to do the Sith use 'My Lord'. So occasionally it's as much an honorary title as one with real substance behind it.
 
It is a lot of personal preference.

Where I started RPing, writers rarely used the Darth title. They went for Lord/Lady or used their chosen Sith name that became their identifier.

I like the sound of Lord Sinistra, so I use that. [member="Dar Moraal"]
 
The Darth name is also remnant of a tradition where the master bestows the title upon the student as a mark of achievement. Ferus was given the name from his master, and I made a habit of giving the titles out to 'worthy students'. Be it Darth or Lord, its often a title given to those of high esteem, as many have said. Ferus would call most by Lord, even if they didn't use a Darth name like [member="Vrag"] . when he did use Darth, it was often used as an antagonistic reminder of their position, and how they should act. Or as a compliment to those who impressed him.

This has all probably been said already, just wanted to toss in my examples with Ferus when he was around.
 
[member="Dar Moraal"] Look for a Latin descriptor and use that: most of the Darth names are either latin derivatives of a quality or concept, or something in English (consider Bane). Vader was German, but the others are all mostly derived from Latin. Have a play around.
 
Latin is always good for names, but you'll find a lot of them being recycled. I took mine from the scientific latin name of an order of animals with which my character shares a theme. Wordplay works nicely, like [member="Darth Hauntruss"]. Canon-based names like Vornskr work well. [member="Darth Pyrrhus"] has his from a historical figure. It really comes down to making a name that resonates with your character and is preferably unused.

As to being granted one's name. From what I've gathered, there is no set tradition common for the whole faction. Ophidia received hers from her master, some simply claim theirs on their own.

For the Dark Lord to come in and give the name is quite special. They say his voice tears eardrums apart by the molecule; his sight boils your blood in the vein and peels flesh from bone. If he sent his thoughts into one's head, they may become catatonic as their feeble minds cannot comprehend the power he holds.

But sure, we may be persuaded to bring him out for something special.
