The titles are earned through roleplaying.

Promotion to Knight Guidelines vary faction to faction that you are with, or Master to Master.
Promotion to Master Guidelines are usually submitted to be voted on, this is also submitted by your Master, either through their faction and/or independently to roleplaying judges.

So you need to find someone willing to train your character and go from there.

You can also self-train with NPC masters, which is what I'm doing. Then once you think your character has reached the knight-level, you can contact an RPJ and they can promote you, if you don't have someone for that already.

So you need to find someone willing to train your character and go from there.

You do not need a master, NPC or otherwise, to achieve the rank of knight - although it certainly helps. You also don't need to do a single training thread, ever, to achieve either Knight or Master.

2. ask for promotion
3. If step 2 fails go back to step 1
4. get promoted and go back to step 1
