Like most Jedi , Yoda built the saber himself with the help of Professor Huyang, a droid who had been a teacher that helped the Jedi build their lightsabers since the beggining of the Jedi Order.
The saber was lost in the Senate Building, when Yoda duelled Darth Sidious also know as, Sheev Palpatine.
