The Tremiru family is one of powerful alchemists and cunning sorcerers extending back centuries, mastering dark and forbidden rituals to gain further mastery over the Force. The First, who's name was lost when the Sith Empire of old fell to the Old Republic, crafted this lightsaber as a memento of the Tremiru estate and privately as a way to keep the teachings stored away. In the form of her own soul being made into the crystal of the blade. With it, and the runes etched into the metal, she was able to survive centuries. Moving from generation to generation and teaching the next head of the family what would have been lost otherwise.
Time wears down all, however, and whoever she was she simply wasn't any longer. The First now acts as a gate keeper, deciding who is worthy of wielding the blade's secrets. Without it, any user who gets past the trapped guard finds it little more than a normal lightsaber. With it, the First imparts her knowledge and assists in crafting and casting spells and rituals. Because of this, the existence of the First is a closely guarded secret by the head of the family. Only those with her approval can lead, and her power helps to make it so.
