Description:
With the "Iron War Project" gaining momentum, Krayzen Dratos, the creator of the project decided to make sure that the project would also begin working on designing arms, and other weapons for OS soldiers, and not only for the One Sith Navy. He immediately ordered for a meeting to be declared, where he, along with One Sith designers worked together to create the GS-Destructor Grenade in order to provide One Sith soldiers with something they can use to fight force sensitives.

The grenade has a smart link system, meaning that it can be activated via remote controlled, or someone that has linked the grenade with the HUD. When the grenade is activated, it instantly emits white phosphorous gas; white phosphorous gas is one of the most lethal gases in the galaxy, and the gas might disrupt the concentration of a force user, and even potentially kill him. This can be exemplified by the fact that white phosphorus will burn the skin of someone, even in armor.

After white phosphorous gas is emitted, a concussion/ sonic blast is emitted, where it has an area of effect of about five meters. It also adds another barrier to the enemy force user surviving the attack.

While it has some strengths, it is not without weaknesses. For instance, armors with environmental seals can be able to block the white phosphorous gas; additionally, those that have armor designed to fight off kinetic blasts can be able to survive the concussion blast unaffected(depending on the armor rating).

Ultimately, the grenade is powerful, and a useful weapon for One Sith soldiers.

As per the template rules, please put this in metric units (kilograms). I think that this is also a bit heavy for a grenade. I'd suggest a weight between 0.3 and 0.5 kilograms.

When exposed to oxygen (and most atmospheres in the Star Wars universe), white phosporus almost instantly ignites. In other words, this grenade would likely cause a brief fireball and/or a lot of smoke.

This may well accomplish the task of disrupting the concentration of a force user, but it's not a stereotypical gas grenade that it is going to only effect their sensory or nervous system.

How is this concussion blast produced? Most concussion grenades use explosives that result in the grenade's destruction.

Please remove this. There any number of situations where the effects of concussion blasts may be altered by a Force user or even a common person.

Is this a physical barrier that we're talking about? Or do you mean that the concussion blast is another reason why this grenade will be more effective?

1) I put it in kg

2) I added that the white phosphorous gas can burn the skin of someone.

3)
Physical
The first one was a fairly standard grenade, used by many factions. It exploded with a small blast radius, releasing a blast of kinetic energy that would kill most organics and damage equipment. A sonic shockwave was expelled in the explosion that stunned anyone right outside the 4 meter blast radius.

I based that part of the paragraph from that wookipidea quote.

3)I removed that part; in terms of the latter, I mean it adds another barrier to them surviving the attack, not a physical barrier.

This is solid conceptually then. We're close to approval.

Please replace "will surely disrupt" to something like "can disrupt" or is "designed to disrupt". In the factory, we generally don't like item descriptions that give an automatic offensive end result, even if it's likely or highly plausible. So this isn't to say that it shouldn't happen usually, but there are always extenuating circumstances (such as the force-user who sacrifices his body to ensure his concentration on whatever technique is working, etc).
