Notification issue (PM and topics)

It's happened a couple of times so far. Basically, the system hasn't notified me of certain private messages or posts in which I was mentioned (I've read the FAQ, but this doesn't apply as one of the topics I'm mentioning actually has my name properly 'mentioned'). If I go to view all of my notifications, it still doesn't show the notification.

I've checked my settings and everything looks good, the problem seems relatively random.
